Pillars of Care Assisted Living Home, LLC provides services that integrate home care with community-based support for individuals who qualify for I/DD Medicaid Waiver Services or who pay privately. Below are answers to our most frequently asked questions about the I/DD

Pillars of Care Assisted Living Home, LLC will create personalized Service Plans to meet each resident's unique needs while respecting their preferences and promoting independence.
Pillars of Care ALH is staffed with dedicated and compassionate CNAs and caregivers, all of whom have years of experience in this environment.
A Registered Nurse is also part of the administrative and support team. While skilled nursing services are not offered, the RN monitors residents’ well-being, addresses health-related concerns, and provides oversight and education to the care providers and staff. Care is available 24 hours a day.
We ensure that all our services are tailored to meet each resident’s individual needs and preferences while respecting their choice, independence, individuality, privacy, and dignity.

We’re here to help you with everyday activities like bathing, using the restroom, getting dressed, grooming, doing laundry, moving around, preparing meals, and much more. Your comfort and well-being are our top priorities!
Making sure your loved one gets the right medication on time and as prescribed. This service also involves requesting refills whenever necessary.
Make Appointment: This will be a casual conversation to learn more about the residence and understand the needs of the individual seeking assisted living services. If requested, this step may also involve a visit to the home.
Application: You can apply for an assessment or refer a relative for one. At this step, you may also receive an application packet.
Assessment: The Home RN or administrator evaluates the applicant’s medical needs, cognitive abilities, and functional capacity.
The assessment takes into account the applicant’s personal, housing, physical, emotional, and mental health needs.
Paperwork: Complete the admission paperwork, which includes a resident or
representative agreement.
The resident agreement is a legal contract that details the services the home will provide and the associated fees for those services.
Eligibility: Applicants must meet the minimum age requirement and need assistance with activities of daily living (ADL).
Applicants must be medically stable and able to move independently. Applicants should not need 24/7 medical care like that required in a medical facility like a nursing home.
This will include the following:
Care Conference: this includes the resident, their representative, the Pillars of Care ALH representative or RN, the resident’s Care Coordinator, and other necessary medical team members. This will also include the development of the resident’s Plan of Care that details the services and ADLs the resident will receive.
Move-In Process: Ensuring a smooth transition for the resident.

In Alaska, assisted living homes (ALHs) are residential facilities that provide nonmedical care for seniors or individuals with disabilities who need help with activities of daily living (ADLs) like bathing, dressing, and eating. Services typically include personal care, medication management, meals, housekeeping, transportation, and social activities. Some ALHs oJer specialized care, such as dementia support.

Medicaid (known as DenaliCare in Alaska) does not cover room and board but can cover personal care and other services through waivers like the Alaskans Living Independently (ALI) waiver or the Adults with Physical and Developmental Disabilities (APDD) waiver. Nearly 1 in 6 assisted living residents nationwide rely on Medicaid for care services, and Alaska’s waivers support this.

To qualify for Medicaid-covered services in assisted living, individuals must be
Alaska residents, meet financial criteria (e.g., income below $2,829/month for an
individual in 2024, or 300% of the SSI Federal Benefit Rate, and assets under $2,000 for a single person), and require a nursing facility level of care. For the ALI waiver, applicants must be 65+ or 21–64 with a physical disability. The APDD waiver targets those 21+ with developmental disabilities. A functional assessment by the Division of Senior and Disabilities Services (SDS) is required.

Apply through the Alaska Division of Public Assistance (DPA) online via the ARIES Self-Service Portal, by mail, fax, email, phone (800-478-7778), or in person at a local oJice. Required documents include proof of identity, income, assets, and residency. After Medicaid approval, contact the SDS Intake and Assessment Unit for a functional assessment to determine waiver eligibility. Applications are accepted year-round, but waitlists may apply due to limited slots (e.g., ALI has limited enrollment).
